Michelle Albrecht,
Physical Therapist
Michelle provides services at our Brandon outpatient clinic, Helping Hand Assisted Living Center and treats patients at Palisade Manor on an in-patient and out-patient basis. Michelle started with Prairie Rehabilitation Services in August of 1999. Michelle has a special interest and training in women's health, such as urinary and fecal incontinence and pelvic floor muscle pain. She also treats patients with musculoskeletal dysfunction related to pregnancy. Michelle has taken a wide variety of additional training for the treatment of orthopedic dysfuncitons and has been certified to provide the ASTYM System. Michelle graduated with a Master's degree in Physical Therapy from the Medical College of Virginia in Richmond, Virginia in 1999. She is a member of the American Physical Therapy Association (APTA), the South Dakota Physical Therapy Association and the Women's Health Section of the APTA. Michelle is also a member of the Brandon Valley Chamber of Commerce and serves on their marketing committee, as well as assisting with the monthly newsletter. |

Charlie Bigelow, Physical Therapist
Charlie is a founding partner of Prairie Rehabilitation Services and provides services in our outpatient clinic in Worthington, Minnesota as well as Maple Lawn Nursing Home in Fulda, Minnesota. Charlie has orthopedic training in manual therapy for the back and neck as well as specialized training in Rehab Medicine Compliance,
neurological rehab, geriatric rehab and Graston. Charlie graduated from the University of North Dakota in 1981. He is a member of the American Physical Therapy Association and the Minnesota Physical Therapy Association. |

Cheryl Butzer, Physical Therapist Assistant
Cheryl provides service at our Brandon clinic, Helping Hand Assisted Living Center, home health in Brandon and treats patients at Palisade Manor in Garretson, on an in-patient and outpatient basis. Cheryl started at Prairie Rehabilitation Services in June 1995. She has a special interest in treating muscle imbalances, headaches, cervical pain, balance rehabilitation, core stabilization and functional rehabilitation. She received her PTA degree in May 1983 from the College of St. Catherine's in Minneapolis/St. Paul, MN. Cheryl has exceptional experience in Orthopedics, geriatrics and pediatrics. |

Andrew Dickhaut, Speech-Language
Pathologist
Andrew provides services to many of our southeast Minnesota long-term
care facilities as well as our Minnesota outpatient clinics. Andrew
started with Prairie Rehabilitation Services in June 2000. Andrew
is certified by the American Speech-Language and Hearing Association
(ASHA) as well as being licensed and authorized to provide the services
necessary for the diagnosis and treatment of speech-language disorders
that result in communication disabilities. He is qualified to provide
dysphagia (swallowing difficulty) assessments and management services.
Andrew is a December 1998 graduate of the University of South Dakota
with a Master's Degree in Communication Disorders. He is also a member
of the American Speech-Language and Hearing Association. |

Sarah Fosheim, Physical
Therapist
Sarah provides services at our Sioux Falls outpatient clinic. She
started with Prairie Rehabilitation Services in 1998. Sarah started
developing the Women's health program when she came to Prairie Rehabilitation
Services. She has special interest and training in women's health,
including treating patients with urinary and fecal incontinence, pelvic
floor muscle pain and musculoskeletal pain related to pregnancy. Sarah
also has a strong interest in muscle energy techniques to treat pelvic
and spinal mal-alignment. Sarah instructs the Women's Health module
at the University of South Dakota Physical Therapy School. She graduated

Kristin began working for Prairie Rehabilitation Services in June of 2007. She provides services at our Worthington outpatient clinic as well as South Shore and Cross Roads Care Centers in Worthington, MN. Pathologist
Gina provides services to many of our southwest Minnesota and southeast
South Dakota long-term care facilities as well as our outpatient clinics. Gina started with Prairie Rehabilitation Services
in September 1997. Gina has special interests in the areas of adult
dysphagia, stroke rehabilitation, rehabilitation of adolescent and
adult head injuries and Parkinsonian voice disorders. Gina graduated
from the University of South Dakota in 1988 with a Master's Degree.
Gina is currently licensed in Minnesota and Iowa and is also able
to practice in South Dakota. She is a member of the American Speech-Language-Hearing
Association and the South Dakota Speech Language and Hearing Association. |

Lee Glasoe, Occupational Therapist/Certified
Hand Therapist
Lee is a founding partner of Prairie Rehabilitation Services and provides
services in our Sioux Falls outpatient clinic. Lee started the Sioux Falls Center for
Hand Therapy in 1991 which later merged with Physical Therapy Associates
to create Prairie Rehabilitation Services. Lee has special interest
and training in upper extremity rehabilitation, work injury and job
site management. He is also certified in Functional Capacity Evaluations and ASTYM treatment. Lee graduated from the University of North Dakota in 1987. He is a
member of the American Occupational Association, South Dakota Occupational
Therapy Association, American Society of Hand Therapists and the National
Association of Rehabilitation Agencies. |

Paula provides home health services in Sioux Falls. Paula started
with Prairie Rehabilitation Services on December 11, 2003. She has
worked in a variety of settings in South Dakota including acute care,
outpatient, long-term care and post secondary education. Paula graduated
from the University of Iowa with a Master's in Physical Therapy in

Al Holm, Physical Therapist
Al is part owner of Prairie Rehabilitation Services and provides services
at our outpatient clinic in Sioux Falls, South Dakota as well as for
Good Samaritan Home Health in southeast South Dakota. Al began working
for Prairie Rehabilitation Services in March 1998 and became a part
owner of Prairie Rehabilitation Services in September 2001. Al has
special interest in orthopedics including shoulder, spine and hand
therapy. He also has
interest in functional capacity evaluations, ASTYM, ergonomics and stabilization programs. Al graduated from the University
of North Dakota in 1982. He received his masters in Human Factors
Psychology from the University of South Dakota in 1999. Al is a member
of the South Dakota Physical Therapy Association and the American
Physical Therapy Association. He was recognized as the South Dakota
Physical Therapist of the Year for 2004 by the South Dakota Physical

Doug provides services at the Good Samaritan Communities of Windom as well as other facilities in Southwest Minnesota as needed. He graduated from Lake Area Technical Institute in May 2007. Doug worked previously with Prairie Rehabilitation Services from May 2007 to November 2007. He returned in December 2007. Doug also holds a bachelors degree from University of Northern Iowa in Health Promotion with an emphasis on Wellness. Doug enjoys working with geriatrics but also has a passion for sports medicine and orthopedics. |

Lori Johnson, Physical
Therapist
Lori provides services at our Sioux Falls, South Dakota outpatient
clinic.
Lori started with Prairie Rehabilitation Services in November of 2001.
Lori has special interests in women's health and low back pain related
to pregnancy. She is also a certified lymphedema therapist and was
the first to treat patients with lymphedema at Prairie Rehabilitation
Service. Lori is a certified Juzo, Jobst and CircAid fitter for compression
garments. Lori graduated from the University of South Dakota in August
2001 with a Master's Degree in Physical Therapy. She is a member of
the American Physical Therapy Association, the South Dakota Physical
Therapy Association and the National Lymphedema Network.
Lori won the Genesis Award in 2006 from the South Dakota Physical Therapy Association for her achievement in developing her professional career in the field of physical therapy. |

Tasha Knothe - Physical Therapist Assistant
Tasha provides services at our long term care facilities in Worthington, Fulda, and Westbrook, Minnesota. Occasionally, she also covers at other surrounding long term care facilities and the Worthington outpatient clinic in Minnesota. Tasha graduated from Lake Area Technical Institute in Watertown, SD with an Associates Degree as Physical Therapist Assistant in May 2005. Tasha began working with Prairie Rehabilitation Services right out of school for a short time before moving to the Twin Cities where she worked for 2 years. Tasha returned to Prairie Rehabilitation Services in September of 2007 to work as a PTA. Tasha has special interests in geriatrics, orthopaedics and aquatic therapy. |

Gidget provides services in many of the nursing homes we serve in South Dakota as well as our Sioux Falls outpatient clinic. Gidget started with Prairie Rehabilitation Services in November of 2000. She has special interests in geriatrics, working with hand dysfunctions, wheelchair positioning and pressure mapping. Gidget is certified in performing Functional Capacity Evaluations as well as being certified in performing ASTYM. Gidget graduated from the University of Mary in 1999. She is a member of the South Dakota Occupational Therapy Association and is a member of their continuing education committee, bylaw, policy and procedure committee and past executive board member. Gidget is also a member of the American Occupational Therapy Association. |

David provides services to the Mary Jane Brown Good Samaritan Center in Luverne, MN;
Edgebrook Care Center in Edgerton, MN; and Pipestone Good Samaritan
Village in Pipestone, MN. David is a graduate of Valparaiso University
in Valparaiso, IN and the University of Iowa Physical Therapy Program.
He has over 35 years of experience in physical therapy and since 1995
he has specialized in geriatrics. David joined Prairie Rehabilitation
Services full time staff in January 2004. He is a member of the American
Physical Therapy Association, the Geriatric Section of the APTA and
the South Dakota Physical Therapy Association. David is an avid bicyclist
and lives in Sioux Falls, SD with his wife Jean. They have three grown
sons and two granddaughters. |

Janelle has been an occupational therapist for 9 years. Her experience consists of geriatrics, pediatrics, orthopedic, and hand therapy. Kathy Smith, Physical Therapist
Kathy provides services to the Golden Living Center in Slayton, MN. She started with Prairie Rehabilitation Services in April 2006. Kathy is a 1972 graduate of the University of Minnesota in Minneapolis, MN and has 35 years of experience in physical therapy. She has worked in acute, subacute, outpatient, home health, school and long-term care settings and especially enjoys working with and helping her geriatric clients. She has additional training for the treatment of mechanical neck and back pain, sacroiliac dysfunction and mobilization of the spine and peripheral joints. Kathy feels blessed and honored to be a Physical Therapist and part of Physical Medicine and Rehabilitation. Kathy has one son, Damon, who is a Certified Prosthetist and works for Arizona Prosthetic and Orthotic Services in Phoenix. He is currently studying to be a Certified Orthotist as well. |

Jeff is a founding partner of Prairie Rehabilitation Services and provides services in our Sioux Falls outpatient clinic. He also provides services at Riverview Manor in Flandreau, South Dakota and to Good Samaritan Home Health care in southeast South Dakota. Jeff started Physical Therapy Associates in 1990, which later merged with the Sioux Falls Center for Hand Therapy to create Prairie Rehabilitation Services. Jeff has special interests in treating muscle imbalances, headaches, and cervical pain as well as working with geriatric patients, neurologically involved patients, wheel chair seating and positioning, and functional rehabilitation. He also has a close working relationship with the MDA clinic. Jeff graduated from the University of North Dakota in 1981. He is a member of the American Physical Therapy Association and the South Dakota Physical Therapy Association. He is also a physical therapy subcommittee member of the South Dakota Board of Medical and Osteopathic Examiners. |

Diane Stoltenberg, Occupational Therapist
Diane joined Prairie Rehabilitation Services in June 2007. She provides services to the long term care facilities and outpatient clinics in Jackson and Lakefield, MN as well as the long term care facilities in Lake Park and Estherville, IA. Diane earned her BS degree in Occupational Therapy from the University of North Dakota, Grand Forks in 1983. Diane previously served the rural North Dakota communities surrounding Jamestown and has experience in hand therapy, outpatient orthopedic and neurological services, inpatient care, long term care, developmental disabilities, home health services, hospice, community ergonomic programs and mental health services. She is currently working towards her certification in cognitive care. After relocating with her family to southern Minnesota in 2005 Diane provided occupation therapy services for pediatric clients and their families through the school system programs. Diane has developed a variety of therapy interests with the consistent goal of assisting individuals and their families to live quality lives with optimal levels of function and well-being. |

Michele provides services at our Sioux Falls and Tea outpatient clinics. Michele started with Prairie Rehabilitation Services in April 2006. She has been working as a registered Physical Therapist since 1995 in orthopedics, spinal cord and brain injury as well as geriatric and neurological rehabilitation. Michele has specialized training in the treatment of the spine, spine and extremity mobilization, MFR and manual therapy. Her areas of special interest include orthopedics and geriatrics. Michele graduated from the University of Florida in 1995 with her degree in Physical Therapy. She is a member of the American Physical Therapy Association and the South Dakota Physical Therapy Association. |

Physical Therapist
Brett provides services at the Canistota Good Samaritan Center in
Canistota, South Dakota, the Tieszen Memorial Home in Marion, South
Dakota as well as our Hartford, South Dakota outpatient clinic. Brett also provides services for Good Samaritan
Home Health Care in southeast South Dakota. Brett started with Prairie
Rehabilitation Services in November 2001. He has worked primarily
in geriatrics for the past few years and has specialized training
in low back care as well as cerebrovascular accidents (strokes). Brett
graduated from the University of South Dakota in July of 1999. |

Donna Thaemlitz, Physical Therapist
Donna provides services at our Lakefield, Minnesota outpatient clinic
as well as Colonial Manor in Lakefield, Minnesota and the Jackson
Good Samaritan Center in Jackson, Minnesota. Donna started with Prairie
Rehabilitation Services in August 1999. She has special training in
joint mobilization of the spine and peripheral joints. Donna has a
special interest in orthopedics and sports therapy and also treats
women's health patients. Donna attended Physical Therapy school in
Rochester, Minnesota at the Mayo School of Health Sciences and graduated
in 1989. |

Troy is part owner of Prairie Rehabilitation Services and provides
services at our Worthington, Minnesota outpatient clinic as well as
South Shore Care Center and Crossroads Care Center in Worthington,
Minnesota. Troy began working part-time for Prairie Rehabilitation
Services in 1996 and went to full-time in 2001. He became part owner
of Prairie Rehabilitation Services in June 2005. Troy has specialized
training in pediatrics and interests in manual therapy, therapeutic
exercise and geriatrics. He graduated from the University of South
Dakota in 1995. |
